Home Care
Our team’ goal is to keep you at home safely and to prevent readmission to the Hospital or skilled nursing facility. Our trained nurses and therapists offer assessment, observation and health education. Our team works together with your provider, to provide total health-related goals and the best quality of life.
Wound Care
From the comfort of home, skilled nurses will assess wounds, and teach wound care. We will work with you and your provider with the goal of healing your wound.
Rehabilitation Care
Physical therapists, occupational therapists and speech language pathologists make up our rehabilitation team. These caring professionals work together with you and your caregivers to improve your overall health and safety. They provide Services to restore function, improve mobility, relieve pain and maximize independence. We can also offer re-hab in home.

Post-Surgical Care
Our staff helps to ensure a smooth transition to the home setting by assessing your post-surgical needs at home. Services may include; education on what is considered normal following surgery and on pain medication and pain control issues. We can also monitor any new or chronic conditions that might be affected by surgery, such as wound care, assessment of the incision for infection and more.
Caregiver Support & Resources
Our home health care primary role is providing the best high-quality patient care as well as educating and supporting the patient’s caregivers. Family, friends, partners and neighbors may all be caregivers. Most do not consider themselves as a caregiver but identify with the primary role they have with the patient. Caregivers are crucial in assuring follow through on communication, compliance and collaboration in all aspects of the patient’s care.
Availability:
Home health care is available Monday through Friday, scheduled on weekends and a registered nurse is on-call 24 hours a day to help answer your health questions.

Our home health care services cover approximately a 30-mile radius from Cokato including, Dassel, Buffalo, Howard Lake, Hutchinson, Litchfield, Montrose, South Haven, Watertown, Winsted and everything in between.
Insurance Coverage:
Most insurers cover skilled home care services. Home health care charges will be billed to you and submitted to your insurance company. Cokato Manor Home Health Care is contracted with several HMO’s. We are able to bill a variety of payer sources such as Medicare, VA, private insurance and Medicaid certified. We also work with you to bill your Long-Term Care Insurance.

Cokato Manor Home Health Care is licensed by the State of Minnesota, certified by Medicare, and is a member of the Minnesota Home Care Association. These regulatory agencies assure you are receiving the highest quality home care.
