Outpatient rehabilitation services are available at Cokato Charitable Trust through our therapy partner, Pro Rehab.

You do not need to live at one of our facilities to receive therapy.

Community members of all ages can schedule outpatient physical, occupational, or speech therapy with a physician referral.

Who Outpatient Therapy Is For

Outpatient therapy may be appropriate for individuals who:

Outpatient therapy is provided onsite at Cokato Charitable Trust, offering a convenient option for residents of Cokato and surrounding communities including Dassel, Howard Lake, Annandale, Litchfield, Hutchinson, and nearby towns.

For many individuals, receiving therapy close to home makes the process easier to manage.

A physician order is required for outpatient services. To schedule an appointment, contact Pro Rehab directly at 952-955-2242.

Therapy Hours: Monday through Friday 8:00 AM to 4:30 PM

Inpatient therapy for residents is coordinated through nursing staff and Pro Rehab as part of the care plan.
