Outpatient and Inpatient Services in Cokato
Rehabilitation services at Cokato Charitable Trust are provided onsite through Pro Rehab. Therapy is available for residents receiving inpatient care as well as community members scheduling outpatient services with a physician referral. You do not need to live at one of our facilities to receive therapy.
Each therapy plan begins with an evaluation and is built around practical goals. The focus is simple. Helping people move safely, manage daily tasks, and communicate clearly. Below is an overview of the therapy services available.
Who We Serve
Skilled nursing care at Cokato Manor may be appropriate for individuals who:

Need 24-hour nursing supervision

Are recovering from surgery or illness

Require medication management and monitoring

Need assistance with daily living along with medical oversight

Qualify for swing bed services after hospitalization
Each resident’s care plan is developed individually, in coordination with physicians, nursing staff, therapy providers, and family members. No two situations are exactly alike.
Physical Therapy
Strength, Balance, and Movement
Physical therapy is available for residents and community members through Pro Rehab at Cokato Charitable Trust.
It focuses on improving strength, balance, mobility, and overall movement so individuals can return to daily routines safely.
Occupational Therapy
Supporting Daily Living Skills
Occupational therapy is available for residents and community members through Pro Rehab at Cokato Charitable Trust.
The focus is on helping individuals manage everyday activities safely and maintain independence at home.
Speech Therapy
Communication and Swallowing Support
Speech therapy is available for residents and community members through Pro Rehab at Cokato Charitable Trust.
The focus is on strengthening communication skills and addressing memory or swallowing concerns when they arise.
Speech therapy may be recommended for individuals who:
- Have difficulty speaking clearly
- Are recovering from stroke
- Experience swallowing challenges
- Notice changes in memory or thinking skills

Therapy Hours: Monday through Friday 8:00 AM to 4:30 PM
Inpatient therapy for residents is coordinated through nursing staff and Pro Rehab as part of the care plan.
